Health Services
D34 is dedicated to promoting the physical and mental well-being of all students within the district.
Attendance
Daily attendance is required by the School Code of Illinois. Student absences for any reason other than illness are strongly discouraged. If your child is going to be absent from school, parents must call the school by 9 a.m. on each day of absence to report and explain the reason for the absence. The school phones have answering machines that can receive messages before and after school hours.
Sick Day Guidelines
To help prevent the spread of illness, the Lake County Health Department provides guidelines to help with your decision on whether or not to send your child to school.
Annual Vision and Hearing Screenings
Vision and hearing screenings are performed annually to D34 students by a certified vision and hearing technician. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor or an exam by an audiologist.
Students are not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous twelve months and the evaluation is on file at the school. If a vision or hearing examination report form is not on file at the school for a student in a mandated grade or group, they will be screened.
Vision and Hearing screenings are mandated by Illinois on an annual basis for the following students:
